Wights Are Often Found in Tombs

If you go to deeper caverns and tombs, you may encounter a Wight, a necromancer-type boss. This enemy may prove difficult to beat because it mostly stays in the air and endlessly spawn skeletons and the undead.

Wights are hard to fight head-on because of their tendency to keep themselves levitating. Having ranged attackers like Archers, Mages, and Sorcerers, can help stagger it until it falls to the ground. It is also note-worthy to use weapons like The Exalted to deal holy-damage.

Medusa is Found in Caliginous Depths

You are in for a rematch against the Gorgon you fought at the beginning of the story. On the way to Dragonsbreath Tower, you can see a cave that houses the gorgon, Medusa.

You will need ranged attackers to draw the gorgon's attention while you attack it from the back. Crawl along its back and keep attacking its head with a weapon imbued with Electric-damage. This will sever the head and preserve it giving you a powerful weapon that petrifies enemies.

Although it is not recommended, if you just want to explore the world without progressing the main quest you can totally do that. Some caves connect regional areas allowing you to pass the border without the legitimate papers.

Most caves are usually found at the end of paths and trails you find on the road. Some caverns will be along mountain ranges that require more skillful exploration.
