Found at Shadow Keep

One Iris of Grace is found in the Shadow Keep. Fast travel to the West Rampart site of grace and keep heading east into the Living Jar room. The Iris of Grace will be in front of an altar with a fleshy enemy guarding it.

Another Iris of Grace is found at the Scadutree Base. Drain the water flooding the Church district, then head to building west of the Sunken Chapel. Follow the hallway to the right until you reach the Tree-Worship Passage. Push onwards to reach a headless Statue of Marika, where you'll find the Iris of Grace.
How to Use Iris of Grace
Give to Jolan to Obtain Swordhand of Night Jolan Spirit Ash

The Swordhand of Night Jolan Spirit Ash is obtained by giving Jolan the Iris of Grace as part of her questline. Of the two choices, the other being to give the Iris of Occultation, giving the Iris of Grace will continue the questline.

The Iris of Grace serves as an important quest item, but can also serve as a consumable that grants you the effects of your equipped Great Rune similar to Rune Arcs.
Take note that the Iris of Occultation is non-reusable and should only be used after you have obtained all quest rewards tied to it in a New Game Plus.
