Players cannot reset or respec their perks in Fallout 4. It means that once you've set your perk, it will be permanent and you won't be able to change it for another perk.

Alternatively, you can use the Console Command to add or remove a perk you want in the game. To do that, open the Console Command by pressing the (~) key and typing the necessary code followed by the Perk ID.
Note that the Console Command is exclusive to PC players only.
Starting players will get 21 bonus points that they can allot to Strength, Perception, Endurance, Charisma, Intelligence, Agility, and Luck, or S.P.E.C.I.A.L. in short. Once set, these cannot be changed unless you use Console Command to alter them.
Perks have required levels, such as the S.P.E.C.I.A.L. stats and Player Level, to unlock them. You need to obtain and spend points and reach certain levels to unlock perks!
You can get points for your perks each time you level up. You can level up by doing different activities, progressing in the story, exploring, killing enemies, unlocking things, and so much more!
All perks are obtainable in the game through level grinding or Console Command. The level you need to get all the perks is Level 271.
If you want to reach that level instantly, use the "player.setlevel [preferred level]" in the Console Command!
