Is Ghoulish Worth It?
Useful in Certain Situations
Ghoulish is only good in some scenarios where you are constantly soaked in radiation or in an area with lots of ghouls as some ghouls will become friendly at max level.
You can consider investing your points in other perks as you will have tons of healing options even in the early game.

How to Get Ghoulish
Unlock Ghoulish at Endurance 9
 Ghoulish is on Endurance Level 9 |
The Ghoulish perk is unlocked when your Endurance is at Level 9. You can start at this level by assigning 9 points to Endurance during the Registration Form at the start of the game. Otherwise, you can get to this level by collecting SPECIAL magazines or bobbleheads and spending perk points upon level up.
After getting to Endurance 9, you're free to spend perk points on ranking up the skill, as long as you are at the required character level to rank up.
