How to Start Hole in the Wall

To start this quest, you will need to finish the Vault 81 side quest first since that quest enables you to enter Vault 81 freely. After doing so, you will need to look for Dr. Forsythe, and donate your blood.

The quest will not start immediately after this as you will need to finish the Here, Kitty Kitty side quest first and wait for 24 hours outside of the Vault, then the continuation of the quest will trigger.

Talk to Dr. Forsythe

Doctor Forsythe is located at the Clinic room in Vault 81.

After riding the elevator, go downstairs from the first stairway. Then, proceed down another set of stairs located just across the hall. On this level, you will find the Clinic. Enter and talk to Dr Forsythe.

Haggle for a Higher Pay

With a high enough level of Charisma, you can haggle for your reward for up to 150 Caps.

Donate Blood and Wait for 24 Hours

The doctor will ask you to sit in one of the chairs. Sit down and he will draw blood from you. He will also provide the promised caps here.

After donating, you will have to wait for 24 hours outside of Vault 81 and then heading back inside to trigger the next step.

Do the Here, Kitty Kitty Quest

Sometimes waiting a day not be enough due to a known bug. A verified fix for this issue is to complete the Here Kitty, Kitty side quest concurrently while waiting.

During this process, wait for 24 hours outside of Vault 81 before returning, which should trigger the continuation of the original quest.

Follow Bobby

Run back to the Clinic and catch the conversation between Dr. Forsythe, Dr. Penske, and Bobby De Luca.

Dr. Penske will then be asking you to follow Bobby to find the source of the Mole Rat that bit Austin. Follow him to a secret passage.

Explore the Secret Vault

Enter the secret vault and proceed downstairs, staying vigilant for enemies such as Mole Rats, Turrets, and Robots. Ensure you unlock every terminal you find to access locked doors.

While the Vault is large, it is fairly linear. Follow the designated path and explore until you reach an elevator.

Instead of taking the elevator, head to the locked room on the right and speak with Curie through the window.

Use the V.A.N.S Perk to Guide the Way

If you have the quest tracked, the V.A.N.S. Perk is especially useful.

This perk allows you to see the next objective when you enter VATS mode, which is particularly helpful in navigating the large Secret Vault room.

Talk to Curie

You won't be able to open the door to Curie's room as this will be opened by Curie herself after having a conversation with her. All dialogue options lead to her opening the door.

After opening the door, Curie will provide you the cure for Austin's illness.

Curie Affinity and Location Guide

Take the Cure Back to Dr. Forsythe

Return to the Clinic to talk to Dr. Forsythe and give him the cure. While you have the option to withhold the cure and let Austin die, doing so offers no rewards.

By giving the cure to Austin, you will complete the quest and receive a Syringer Rifle along with access to a Vault room. Additionally, Curie will become available as a companion.
