Yes, It Boosts Your Damage in Early Game

Bloody Mess is a good perk to have in early game, since it slightly adds bonus damage to your enemies whenever you attack them. On paper, the bonus may not be that much, but in the early stages of the game, your weapons won't be that strong yet and the Bloody Mess perk should more than enough make up for it.

Rank Required Lvl. Description
Rank 1 - The +5 percent bonus damage means enemies will sometimes explode into a gory red paste.
Rank 2 9 You now inflict +10 percent damage in combat.
Rank 3 31 You now inflict +15 percent damage in combat.
Rank 4 47 When an enemy explodes, nearby enemies may suffer the same fate.

Bloody Mess is on Luck Level 3

The Bloody Mess perk is unlocked when your Luck is at Level 3. You can start at this level by assigning 3 points to Luck during the Registration Form at the start of the game. Otherwise, you can get to this level by collecting SPECIAL magazines or bobbleheads and spending perk points upon level up.

After getting to Luck 3, you're free to spend perk points on ranking up the skill, as long as you are at the required character level to rank up.
