Only if You Use Silenced Weapons

While the main draw of Mister Sandman is the ability to instantly kill someone in their sleep, it also grants you bonus sneak damage with silenced weapons, so it's definitely worth picking up only if you use guns with silencers and prefer a stealthy playstyle.

Otherwise, you can skip this perk if you're the noisy type who charges in guns blazing. Unless of course, you also enjoy killing NPCs in their sleep, then by all means grab this perk!

How to Get Mister Sandman

Unlock Mister Sandman at Agility 4

Mister Sandman is on Agility Level 4

The Mister Sandman perk is unlocked when your Agility is at Level 4. You can start at this level by assigning 4 points to Agility during the Registration Form at the start of the game. Otherwise, you can get to this level by collecting SPECIAL magazines or bobbleheads and spending perk points upon level up.

After getting to Agility 4, you're free to spend perk points on ranking up the skill, as long as you are at the required character level to rank up.
