The Mysterious Serum has three effects and one of them is to remove Rads, -36000 to be exact.

The Rads removal is not an instant effect as it will only remove 10 Rads per second, to a total of 1 hour.

Swim in Irradiated Waters

You can use the Mysterious Serum to swim in irradiated waters without worrying about Radiation damage when you have the Ghoulish perk maxed out.

Increase Strength and Carrying Capacity

The Mysterious Serum perk also gives you a +5 in Strength which proves useful as it also increases your Carrying Capacity at the same time.

Use the serum whenever you plan on farming materials or scavenging items.

Mysterious Serum Effect Lasts for 20 Hours

The effect of the Mysterious Serum is not permanent but it takes a long while to wear off.

The 20 hours duration can be reduced via in-game time by means of waiting, fast travelling, or by playing the game normally.

Increase Damage Reduction

The Mysterious Serum also provides +50 Damage Reduction, which is helpful when fighting strong enemies or evern hordes of regular enemies.

Same as the Strength boost though, this effect only lasts for 20 hours.

Special Delivery Quest

The Special Delivery is a quest given by Edward Deegan once you reach a certain point in the game. In this quest your goal is to defeat raiders and retrieve the Mysterious Serum from them.

You can keep the Serum by lying to Deegan although you will need to increase your Charisma to persuade him about it.

The Secret of the Cabot House Quest

In The Secret of the Cabot House quest, you will have to kill the Cabot family and save Lorenzo.

As a reward for saving him, he will offer you a lifetime supply of Mysterious Serums.

Press the tilde (~) key to open the console and type the following code followed by the quantity of the item you'd like to have:
