Is Cannibal Worth It?

Better to Stock Stimpaks

The Cannibal perk lets you feast on corpses to restore a bit of your Health, which can be done better by Stimpaks. Stimpaks are extremely common in all areas, and you shouldn't run out of Stimpaks even after a few encounters.

It's better to invest in other perks instead of Cannibal to increase your capabilities further as you progress in the game.

How to Get Cannibal

Unlock Cannibal at Endurance 8

Cannibal is on Endurance Level 8

The Cannibal perk is unlocked when your Endurance is at Level 8. You can start at this level by assigning 8 points to Endurance during the Registration Form at the start of the game. Otherwise, you can get to this level by collecting SPECIAL magazines or bobbleheads and spending perk points upon level up.

After getting to Endurance 8, you're free to spend perk points on ranking up the skill, as long as you are at the required character level to rank up.
