There are a total of three modes of difficulty. Two of them, Story and Action focused mode, are available upon playing the game the first time. Final Fantasy Mode only becomes available once you've beaten the game.

Story Focused is a mode where players start with TImely Accessories equipped to help them out in battle with automatic reactions during combat. Enemies will have lesser HP and have weaker attacks, allowing players to defeat them quickly and progress the story faster.
In the Story Focused Mode, tools that make FF16's fast-paced combat system easier called the Timely Accessories are enabled.
One of these accessories slows time down you're about to get hit by an enemy attack, allowing you to react and evade via Quick Time Events. Another allows you to execute complex ability combinations by pressing a single button. One of the accessories even fully automates evasion altogether!

The Action Focused Mode is the standard mode of the game, meant for players who are comfortable with action games.
In this mode, the Timely Accessories that make gameplay easier are not automatically equipped. However, the player can still equip a combination of these accessories in-game in case they want to adjust the difficulty to their liking.

Final Fantasy Mode only becomes available in New Game Plus upon completing the game in either of the two modes. In this mode, enemies have higher health and damage, and it caps Clive's level to only level 100.
You'll be able to carry over your unlocked Abilities, your Level and EXP, Gil, Items, and Equipment. The main thing to focus in Final Fantasy Mode is crafting an Ultima Weapon, which is only available in this mode.
Difficulty modes can also be changed in-game via the settings menu. Go to "Menu" > "System" > "Game Settings", and change the gameplay mode from there. Do note that players cannot switch between modes during combat.
List of Controls and Best Settings to Use
After starting a saved file on Final Fantasy Mode post-game, you will not be able to change the difficulty on that file.
Upon playing the game for the first time, you'll be prompted to choose between Story or Action Focused before proceeding. Final Fantasy Mode will not be available here since you've only just started playing the game.
