Dropped by the Death Knight in Fog Rift Catacombs

Death Knight's Twin Axes are obtained by defeating the Death Knight boss at the end of the Fog Rift Catacombs. The catacombs will be filled with deadly traps and winding halls. Make sure you have enough Flasks by the time you reach the boss room, otherwise you will have to reset from the start or from the Statue of Marika midway through the dungeon.

You can quickly get to the Fog Rift Catacombs from the Castle Front site of grace by heading north through the valley guarded by Messmer's Soldiers. You'll know you're on the right path if you reach a large graveyard filled with Shadow Militia (after passing the soldier camp).
