A Misericorde getting lowered base damage and scaling damage from upgrading to +25
A bug has been discovered where all keen affinity weapons gets lower damage output at max rank. Be wary of you're playing a Dexterity focused as the bug only persists for the keen affinity type and all other affinities are unaffected.
With Fromsoft's latest patch update on Mar 17, all Keen Max upgrade lowered damage are now fixed. Damage output of these upgrades are now increased properly.

In order to enhance your weapons, a smithing table is required. These smithing tables are found in the open world and finding one is fairly easy. The first smithing table that you can encounter is found at the Church of Elleh. It is to note that you can only upgrade your weapons to +3 at these tables, to upgrade your weapons further will require the help of the blacksmith.
To use a smithing table to upgrade your weapons, make sure you have the necessary materials needed. Once you have everything, approach the table and press the interact button.
Church of Elleh Location and What to Do
Smithing Master Hewg and Blacksmith Iji can upgrade your weapons to +10 or +25 depending on the type of weapon you are upgrading.
Normal type weapons can be upgraded to +25 while Special type weapons can be upgraded to +10. Both weapon types will require a specific kind of Smithing Stone to ehance.
Smithing Stone is the material required to upgrade your weapons and depending on the weapon, it will require a specific kind of Smithing Stone to ehance.
There are two types of Smithing Stones, the standard Smithing Stones are used to upgrade your Normal weapons while Sombering Smithing Stones are used to upgrade Special weapons. They can be found and dropped by enemies in the open world or purchased at specific merchants.

Hewg can be found at Roundtable Hold. Whether you've defeated or fought Margit the Fell Omen once, rest at a Site of Grace and speak with Melina. She will then offer to take you to the Roundtable Hold.
Upon arrival to the Roundtable Hold, head to the door directly at the opposite end of the great table. After going through the door you should be able to find him to your left.
Ashes of War are equippable skills that can be equipped to weapons. Just like at any Site of Grace, if you have the item Whetstone Knife you can customize your equipped Ash of War at the blacksmith.
The Ash of War skill that you can equip will vary and depend on the weapon you are using, some skills are only availabe for Straight Swords while other skills can be used for any weapon.

The affinity of your weapon can be changed with the item Whetblade, the different affinities that can be selected will be dependent on the type of Whetblade you have in your inventory.
The Iron Whetblade for example can add Keen, Heavy, and Quality affinity options.
Owned Ashes of War can be duplicated by Hewg in exchange for Lost Ashes of War.

Iji can be found behind a large anvil at Northern Liurna, near the Road to the Manor Site of Grace.
Iji offers you his blacksmithing services but unlike at Hewg's smithy, you cannot change nor duplicate your Ashes of War. Customizing your weapon's affinity is also not available at his smithy.
To compensate for not being able to work with your Ashes of War, Iji has a store that sells Sombering Smithing Stones for you Special weapons.
