Found Behind an Illusory Wall in Lamenter's Gaol

The Lamenting Visage can be found on a corpse behind an illusory wall in Lamenter's Gaol. You would first need to obtain the Gaol Upper Level Key in order to get the Gaol Lower Level Key, which unlocks the area behind the illusory wall.

How to Get the Lamenting Visage

Lamenter's Gaol can only be accessed by passing through the Dragon's Pit and the Grand Altar of Dragon Communion. From the Grand Altar, head west and down the stream. Keep heading west and you will eventually reach the Charo's Hidden Grave site of grace.

Travel northwest, past the Death Rite Bird boss area, and keep moving forward while hugging the left cliff until you find the flowery path to the Lamenter's Gaol entrance.

From the Lamenter's Gaol site of grace, follow the tunnel until you run into a locked gate that can only be opened with the Gaol Upper Level Key. Since there's no other path for now, turn left and look for its key first.

Just before the narrow stone path, look to your right to find a room with a chest inside. Open the chest to obtain the Gaol Upper Level Key.

Backtrack to the locked gate from earlier (shown in step 2) and use the key to open it. Climb down the ladder past the gate to find a large room with jars and shrieking Jar Innards.

As you move towards the north side of the room, you will run into a Jar Innard crawling out of a jar. From the spot where you found it, look to your east and go up the path that you see. At the top is another Jar Innard and a doorway. Defeat the enemy before going through the doorway.

From the last Jar Innard encounter, move two rooms to the east. The floor on the second room will collapse as soon as you step foot at its center and drop you to the room below.

Exit the room and go up the next sets of stairs. The very first doorway that you pass by (after the first set of stairs) is where you will use the Lower Level Key so take note of this area.

Before you reach the locked room at the very top, turn right to find a tunnel. The room at the other end of this tunnel is the Living Jar-filled room where the next key is.

From the higher ground, you can spot the chest containing the Gaol Lower Level Key if you look to the southwest. Defeat the Living Jars guarding the chest so you can loot it safely.

Return to the room with the stairs and enter through the tunnel that you passed by earlier. The path that you're looking for is immediately after the very first set of stairs.

Go to the end of the tunnel and attack or roll into the illusory wall past the next doorway. Open the revealed gate using the Gaol Lower Level Key to find the room with the Lamenting Visage.
