Found in a Chest Underneath the Tombsward Ruins

From the Church of Pilgrimage in the Weeping Peninsula, head south west until your reach the Tombsward Ruins. Go down the stairs to reach the room guarded by 2 Pages. The chest with the Winged Scythe should be behind the shut door of the large room.
