The Carian Thrusting Shield can be obtained from a Fire Knight in Shadow Keep Church District. Starting from the Storehouse, Back Section Site of Grace, head west until you see the Fire Knight facing the wall.

Only Available in Shadow of the Erdtree DLC

The Carian Thrusting Shield is only available if you've purchased the Shadow of the Erdtree DLC. This weapon is a DLC exclusive and will not be available if you only own the base game.
