How to Get Old Pocketwatch
Obtain After 3 or More Expeditions
The walkthrough team reported randomly obtaining the Old Pocketwatch after doing 3 or more runs, regardless of whether it was completed successfully or failed. So just keep playing, and you'll eventually get the relic.
How to Use Old Pocketwatch
Unlock the Duchess
Take the Old Pocketwatch to the Hooded Figure at the Roundtable Hold. Go through the dialogue until you can choose the bring up the Pocketwatch option. This will trigger a short cutscene and unlock the Duchess afterwards.

The Old Pocketwatch can be used in Relic Rites like any other relic to boost base stats or add effects to a character. It applies a buff where FP is restores upon successive attacks, and adds a +3 to the character's Dexterity stat.
