Elden Ring Nightreign - Prerequisite

In order to start the process to unlock the Duchess, you need the Old Pocketwatch item which can only be acquired randomly via finishing runs.

Acquire Old Pocketwatch By Completing Runs

Elden Ring Nightreign - Acquire Old Pocketwatch

You can get the Old Pocketwatch at random by finishing runs. If you do not get it the first time, you can keep playing until it eventually drops at the end of your runs.

Take note that you do not need to win your runs and can gain the Old Pocketwatch even from lost attempts.

Take Old Pocketwatch To The Hooded Figure

Elden Ring Nightreign - Give Pocketwatch To Priestess

Once you have the Pocketwatch, you can take the item to the Hooded Figure at the Roundtable Hold.

Engaging in dialogue with her will allow you to bring up the Pocketwatch as a dialogue option.

Hand The Pocketwatch To The Figure

Elden Ring Nightreign - Duchess Unlock

Handing the Old Pocketwatch to the Priestess will trigger a short cutscene.

Afterwards, the Duchess will become a permanent addition of your roster when choosing characters to start runs with.
