How to Get Madness Weapons
Guaranteed Drop from Towers with the Eye of Madness

Ruined towers with the Eye of Madness are random events that have a chance to spawn at the start or in the middle of an expedition. If you kill all the worshippers around and at the top the tower, you are guaranteed to get a Madness weapon like Vyke's War Spear or a Frenzied Flame Seal, as well as a Dormant Power drop.
These towers appear on your map as a large red circle. Approach them with caution since the Eye of Madness will keep flaring, causing Madness buildup whenever you're in its line of sight. Tank through the damage using flasks and focus on taking out the worshippers. Kill them all to despawn the eye and reap your rewards.
Has a Chance to Drop from Camps with the Madness Element
Madness weapons have a very small chance of dropping when clearing out Main Encampent POIs that have the Madness elemental icon. Defeat the Frenzied Flame Troll boss to get its dormant power drop.
Note: The chance to get Madness weapons from Frenzied Flame Encampaments is extremely low. If you're struggling to get Madness weapons, try pivoting over to a spellcaster class since it is easier to farm Sacred Seals that can roll with Frenzied Flame incantations.

Of the 3 Madness buildup weapons in the game, only Vyke's War Spear can actually cause decent Madness buildup in melee.
If your team needs a way to deal Madness buildup (e.g., against Libra), don't rely on getting madness weapons. Instead, bring along a caster class (Recluse or Revenant) since the chances of getting Frenzied Flame Incantations are higher than getting this spear to drop.
Madness is a Status Effect and Not an Elemental Affinity
Unlike weapons with an elemental affinity (e.g., Fire, Magic, Holy, and Lightning), there are no weapons with a Madness "element". Madness in Nightreign is a status ailment similar to Blood Loss (Bleed) or Poison.
However, unlike those two status ailments, there are no non-unique weapon skills that cause Madness buildup. Therefore, regular weapons cannot drop with inherent madness buildup, limiting your choices to the 3 uniques listed above.
