Eye Surcoat can be obtained by picking it up on the ground after defeating Rykard, Lord of Blasphemy. It can be found on the spot where Rykard is defeated.

Rykard is found deep inside Volcano Manor, at the top of Mt. Gelmir, in Altus Plateau. The boss room can only be accessed via a waygate portal on a balcony, right after heading up a staircase with the ball-headed serpent enemy.
