Near the Giants' Gravepost Site of Grace

To reach the map fragment, you must head to the topmost part of West Mountaintops of the Giants and circle back down to the east. Head down south and find a way up the mountain while moving past the enemy barricades of Guardians' Garrison.

Cross a giant chain bridge and you will find the map fragment beside a Site of Grace at the other end.
