Imbibing the nectar is required to progress optional questlines in the Elden Ring DLC Shadow of the Erdtree. Read on to learn how imbibing the nectar affects Thiollier's and St. Trina's questline, what happens if you do or not, and more!

Imbibing the nectar multiple times is required to progress Thiollier's and St. Trina's questlines. Choose not to imbibe the nectar and leave will not do anything except halt the questlines.
Imbibing the nectar allows you to progress both Thiollier's and St. Trina's questline. You will need to imbibe the nectar at least four times even if it kills you instantly each time.
After the fourth time you imbibe the nectar and the screen fades to black, you'll hear St. Trina's voice telling you something about Miquella. You can then talk to Thiollier to tell him about St. Trina's words and advance his questline.
Note: Since you'll be dying multiple times, make sure to spend your runes before undertaking this loop. You may end up losing your runes during the quest if you forget to recover them after dying each time.
Thiollier Questline and Location
After telling Thiollier about St. Trina's words and exhausting his dialogue, you will have to imbibe the nectar two more times to hear more dialogue about Miquella from St. Trina.
If you don't imbibe the nectar, nothing happens and you won't get to progress both Thiollier's and St. Trina's questline. You will not lose anything by imbibing the nectar, except for the necessary deaths in the process.
All DLC NPC Quests and Locations
St. Trina will be located inside the Garden of Deep Purple at the Stone Coffin Fissure legacy dungeon. Players will need to unlock this area first and defeat the Putrescent Knight to interact with St. Trina.
Stone Coffin Fissure Location and Walkthrough
Interacting with St. Trina in the Garden of Deep Purple will bring up the option to “imbibe nectar”. You will have to interact with her again to imbibe the nectar after dying each time.
You cannot imbibe nectar without dying. As Thiollier will have told you during the quest, drinking St. Trina's nectar will most likely kill you.
