Obtained via the Remembrance of Midra, Lord of Frenzied Flame

The Greatsword of Damnation can be obtained by beating Midra, Lord of Frenzied Flame and securing their Remembrance. You can then turn in the Remembrance to Enia at the Roundtable Hold to get the Greatsword of Damnation.

Note: Boss Remembrances only grant you one of the two items on use. If you want to get both, make sure to duplicate the Remembrance using one of the many Walking Mausoleums found in the game.

Description
Golden greatsword that once pierced the body of Midra, master of the manse. Used by the hornsent in the execution of a damnation like no other. The barbs that now enwreathe the blade pierce the victim from within, inflicting continuous wounds to assure eternal agony.
