Grafted Dragon Location

Obtained by Trading the Remembrance of the Grafted With Enia

The Grafted Dragon can be obtained by beating Godrick the Grafted and getting the Remembrance of the Grafted. You can then turn in the Remembrance to Enia at the Roundtable Hold to get the weapon for 2000 Runes.

Godrick is fought at the end of Stormveil Castle in Limgrave. You can either take the safer but longer route by going west from the main gate and making your way through the castle interior or storm the castle front and fight through its most heavily guarded areas.
