Forager Brood Cookbook 5 is obtained by interacting with the Kindred of Rot in the swamp north of Moorth Ruins. Start from the Moorth Ruins Site of Grace and go through the cave with the Giant Miranda Sproutto reach the swamp.
Note that you won't be able to obtain Forger Brood Cookbooks if Moore dies during his quest line.
