First, make your way to the north of Altus Plateau, past the Minor Erdtree. Then circle around to the south west, discovering Sites of Grace along the way.

Keep on going until you reach the spot on the map marked with an X. Climb the ladder there to the top. Keep on making your way westward, hopping onto rock bridges and climbing more ladders.

The map fragment for Mount Gelmir will be found along the path, in a cave-looking structure.
