The Serpent Crest Shield can be obtained from the ghostly figure on a chair who can be found east of Belurat, Tower Settlement. You'll need to pick up the Incursion Painting first from the Artist's Shack located southeast of the Cliffroad Terminus Site of Grace.
The Serpent Crest Shield is only available if you've purchased the Shadow of the Erdtree DLC. This weapon is a DLC exclusive and will not be available if you only own the base game.
