To enter the Mountaintops of the Giants, you must obtain the Rold Medallion from Melina after defeating Morgott, the Omen King in Leyndell, Royal Capital.

Use the medallion in the Grand Lift of Rold and you will be taken to the first area. Move forward a bit into the area and you will find the map fragment along the path.
