From the Minor Erdtree (West Mountaintops of the Giants), head north across the bridge. Stargazers' Ruins is down the slope and to the north.

Look for the lone jellyfish in the middle of the ruins. Summon the Spirit Jellyfish Ashes (whose name, by the way, is Aurelia) to gain access to a cellar with a treasure chest containing the Primal Glintstone Blade Talisman.
