You can download the Japanese language audio DLC from the Nintendo eShop for free if you purchased the game outside of Japan.
If you purchased SMT 5 with a Japanese Nintendo account, the game will be playable only with Japanese voices and text.
Atlus has announced previously that this would be the case and you can now enjoy both the English and Japanese audio according to your preference.
To change your audio language, you will need to be in the game's title screen and then go to options as there is no way to change it while you're playing.
Be careful especially when starting a new game if you with to play with a different audio as you cannot change it in-game. However, you can save your game and reload it in a different language and the change will still apply.
