Dropped by Imps with Hatchets in DLC Catacombs

The Forked-Tongue Hatchet is a random drop from Imps wielding Hatchets inside DLC catacombs. You can farm for them at either the Fog Rift Catacombs, Scorpion River Catacombs, or Darklight Catacombs.

Forked-Tongue Hatchet Farming Route

The easiest place to farm the Forked-Tongue Hatchet will be in the Scorpion River Catacombs in the Rauh Base since it has 1 guaranteed Hatchet-wielder spawn that's slightly near the site of grace.

From the Scorpion River Catacombs site of grace, head southeast until you reach a room with a Bigmouth Imp.

After killing the Bigmouth Imp, turn northeast and go down the stairs. This should take you into a large room with a Basilisk Eye Statue on the far end.

Do not cross the room. Instead, go inside the small room to your north (just after you go down the stairs). Inside there will be an Imp wielding a hatchet that you can kill.

Rinse and repeat the process until you get the Forked-Tongue Hatchet.
