You can find the Spear Talisman inside a treasure chest in the room with the large demi-human in Lakeside Crystal Cave. You can find the cave west of the Liurnia Lake Shore.

How to Use Spear Talisman

Attack While Enemy is Attacking

The increased damage of the Spear Talisman will activate when you attack an enemy that is in an attack animation while you're using a weapon with a thrust attack.

While this usually means thrusting swords, there are certain other weapons that do have thrust attacks like the thrust poke of the Greatsword.
