Most of the content designed for PvP leaderboard placements are pay to win (P2W). Diablo Immortal's PvP endgame upgrades that determine how competitive your build becomes are locked behind in-game currency.

Spending real money is required for Awakening Legendary gear, and nearly required for acquiring Legendary Crests, Legendary Gems, and even upgrading normal gems (past a certain point). Free to play players are given one Legendary Crest per month which can be used to acquire Legendary Gems, but remember that you'll need multiple copies of them for upgrading. Keep this in mind if you or your friends are hardcore players looking to be competitive in Diablo Immortal.
