How to Get the Level Boost Materia

Obtained on the right side of the elevator in Souther Ridge - Trailhead

After entering the facility that leads to the Mako Reactor in Nibleheim, use the stairs that heads to the elevator on the left side and the blue materia should be easy to spot.

Buy from the Moogle Emporium

You can also buy the Level Boost materia from the Moogle Emporium once you reach Merchant Rank 6. You can do this by completing Moogle Intel from 6 different regions.
