To prevent Fior mes Iverno from burning during the main quest Ancient Soil, you have to kill all the Steel Garrote at Waterfall Cave. It's recommended to do so before starting Ancient Soil, so you don't lose out on all the merchants and bounties within Fior mes Iverno.
Discovering the Steel Garrote's operations in the Emerald Stair is part of the side quest Steel Discovery. The side quest begins in the Ranger Headquarters on the east side of the map, and unfolds until the Steel Garrote conspiracy and base of operations are revealed.
However, killing all Steel Garrote at Waterfall Cave is merely an optional objective of the side quest. If you do the side quest without doing the optional objective, Fior mes Iverno will still burn during Ancient Soil.
Once you see the city after exiting Naku Tedek, you'll be able to tell whether you managed to save it or not. If the tallest tower in Fior mes Iverno remains intact (emitting a purple light), then the city is saved. However, if the fires are large and the tower's purple glow is nowhere in sight, then that means the Steel Garrote have razed the city.
