Key quest decisions will cause you to miss out on some uniques in Avowed. Listed below are the unique items you can miss out on if you make certain decisions during certain quests.

One Last Trick	Quest: Steel Resolve Steel Resolve automatically fails if you finish the entire Naku Tedek sequence during the main quest, Ancient Soil. This locks you out of getting the weapon.
Carnet’s Cutlass	Quest: Debt of Blood This quest only triggers if you let Fior mes Iverno burn. Saving it locks you out of the quest and the Carnet's Cutlass reward.
Tree of Life	Quest: Emerald Stair Bounties Failing to complete or turn in all bounties before Fior mes Iverno burns in Ancient Soil will lock you out of getting this shield.
Star of Unbeing	Quest: Shadows of the Past Looted from a table inside Naku Kubel. Missable since the trial areas will no longer be accessible after the quest. Severing the adra will only grant you access to the larger chambers but not the trial areas where this is located.
Nimanna’s Ward	Quest: Shadows of the Past Looted from a table inside Naku Kubel. Missable since the trial areas will no longer be accessible after the quest. Severing the adra will only grant you access to the larger chambers but not the trial areas where this is located.
Umpara’s Spear	Quest: Heart of Valor Can only be obtained if you refuse to give the heart to Keipo at the end of the quest. Report Keipo's condition to Chiko to get the spear.
Limb-Taker	Quest: A Cure for Rage Helping Grakohr find a cure will lock you out of getting this great axe. You need to attack and kill him to get this reward.
Bastion of Solace	Quest: Our Dreams Divide Us Still Saving Solace Keep during this main quest will allow you to collect the shield as a reward from Mihala afterwards.

Blackwing Armor	Quest: An Untimely End Unobtainable if you decided to spare Ygwulf during the main quest, An Untimely End.
Stormdancer’s Scale	Quest: Heart of Valor Can only be obtained if you refuse to give the heart to Keipo at the end of the quest. Report Keipo's condition to Chiko to get the full Stormadancer's set.
Gambeson of the Grand Empire	Quest: Escape Plan If you fail to help Leoflaed and Glaedwine during the side quest, Escape Plan, they will not be able to reward you with this unique armor piece once you reach Thirdborn in Shatterscarp.
Berserker Scale	Quest: Wardens Warding Obtained by successfully parlaying with Amalia at the end of the side quest. It can also be looted from the chest by obtaining the key from her corpse after killing her.
Councilor's Garb	Quest: Our Dreams Divide Us Still Can only be obtained if you kill Kostya when you encounter him trying to sabotage Solace Keep.

Vindictive Band	Quest: Cabin Fever Killing Tazaraak (the "friendly" Xaurip near Dehengen's Cottage) before starting the quest will result in Dehengen becoming unresponsive, causing you to lose out on this ring.
Enchanted Suolenet	Quest: Lost Suoles Demanding payment from Miteno at the end of the quest will result in getting the Celestial Loop instead of this amulet.
Celestial Loop	Quest: Lost Suoles Not demanding payment from Miteno at the end of the side quest will result in getting the Enchanted Suolenet instead of this ring.
Night-Mistress' Favor	Quest: A Lady Never Tells Playing as an Arcane Scholar allows you to confront Giuliana about the shipment you deliver. If you threaten to expose her, you will lose out on getting this item.
The Fantastic Alembic	Quest: Precious Light Missable if you returned the Luminous Adra Crate to Captain Bardatto and the Vailian Trading Company.
Berath's Blessed Ward	Quest: Elder Offerings Returning the stolen food during this side quest will prevent you from getting this trinket.
Chanter's Panpipes	Quest: Memory of the Deep Handing over the Mysterious Meteorite to the Giftbearer will reward you with the Statuette of Lament instead of this trinket.
Statuette of Lament	Quest: Memory of the Deep Handing over the Mysterious Meteorite to Josep Abaccio will reward you with the Chanter's Panpipes instead of this trinket.
Forager's Friend	Quest: Food Theft Can only be obtained if you returned the stolen food to Sielo Piannini and you chose not to ask for compensation at the end of the side quest.
Revenant Bell Collar	Quest: The Animancy Method If you do not manage to sell your corpse to Elia during this quest, then you'll miss out on getting this trinket.
Ring of Minor Deflection	Quest: The Animancy Method Can only be obtained if you fight and kill the Delemgan Queen. Resolving this encounter peacefully locks you out of getting the ring.
Ring of Pain Amplification	Quest: Nature vs. Nurture If you resolve things peacefully with Amadio, then you'll lose out on getting this ring.
Ring of the Peerless Marksman	Quest: Missing Rangers Missable if you tell Fidelio to seek help from the Animancers when confronted about what to do with Belenna.
Heart of the Leviathan	Quest: Heart of Valor Can only be obtained if you refused to give the heart to Keipo and decided to keep the heart for yourself at the end of the side quest.
Stormdancer Ring	Quest: Heart of Valor Can only be obtained if you refused to give the heart to Keipo at the end of the side quest then informed Chiko about Keipo's condition.
Thirdborn Tira Nui Badge	Quest: The Wasteland Courier Can only be obtained if you hand over the Suspicious Package to Tira Nui Hajime in Thirdborn.
Stoneheart	Quest: A Cure for Rage Can only be obtained if you help Grakohr secure a cure for his illness. Killing him prevents you from getting this amulet.

During certain quests, you will be asked to if you want to receive the power of the voice in your head. Refusing to do so will cause you to lose out on certain Godlike Abilities.

Thorn of Sapadal	Region: Emerald Stair Missable Trigger: Won't be granted if you refuse the power of Sapadal during the main quest, Ancient Soil.
Severed Branch	Region: Shatterscarp Missable Trigger: After accepting Sapadal's power in Shadows of the Past, choosing to destroy the ruins will cause you to get Scion of the Immortal Land instead of this Godlike ability.
Scion of the Immortal Land	Region: Shatterscarp Missable Trigger: After accepting Sapadal's power in Shadows of the Past, choosing to sever the adra will cause you to get Severed Branch instead of this Godlike ability.

Naku Tedek Sequence in Ancient Soil is a Point of No Return

During the main quest, Ancient Soil (2nd main quest in Emerald Stair), you will need to explore an underground Godless Ruin in Naku Tedek. If you finish this sequence, you will enter a point of no return for the main story since it advances certain story beats as soon as you exit.

Make sure that before you descend into Naku Tedek, you complete all the side quests in both Dawnshore and Emerald Stair that you've been putting off.

How to Save Fior mes Iverno

Entering the Garden is the True Point of No Return

In the quest, Our Dreams Divide Us Still, your final objective will be to Enter the Garden. This is the true point of no return as you will no longer be able to return to the base game to do side quests and explore.

Since Avowed has no new game plus, this is the start of the end of your run should you proceed. Make sure to create a save before entering the Garden, otherwise you'll have to start an entirely new game in order to tackle side content.
