When consulting Elia during the Animancy Method, ask "Where do all of these corpses come from, anyway?" followed by "Would you be interested in buying my future corpse?". Note that picking any other option after asking where the corpses come from will lock you out from getting the accessory.

Proceed with the main quest afterwards. After putting Silvo to rest, speak to Elia again and she will give the Revenant Bell Collar along with some coins at the end of the conversation.

Note that if you eliminate Silvo before Elia asks you to, a new option will appear mentioning that you already dealt with him. Do not pick that option as the conversation will end and opt you out of getting the accessory.

How to Summon the Revenant

Use the action radial and select the Revenant Bell Collar to summon Eld Maegy. Eld Maegy will remain in the field for at most 35 seconds before disappearing.

Only one revenant can remain active in the field. After vanishing, Revenant Bell Collar will enter cooldown for 60 seconds, where summoning Eld Maegy is disabled until it runs out.
