Steel Garrote Medallion can be found inside the Ruinous Cavern in Dawnshore. The entrance to the dungeon can be found along the western wall, outside of Paradis.

After entering the first large room, turn right to find a passage blocked by webs. Burn the web, follow the path, and loot the corpse at the end to acquire the amulet.

Travel along the outer western wall of Paradis to reach the entrance of the Ruinous Cavern.

Avowed - Turn Right and Burn the Web
Traverse the cavern until you arrive at the first large room with hostile spiders present. Turn right and burn the web with fire damage or Kai's Firestarter to unlock a path.
