During Chapter 3, when searching for the exit in the Mythril Mine, you can find one on the gate that opens up after pushing a mythril cart.

During Chapter 4, you can go to the 2nd floor of Junon Inn and find the fireplace. The Materia can be found right beside it.
The Lightning materia can be obtained from the following combat simulator stages.
