The Sunken One is located in Shetcombe Farm northeast of Kvatch. Find and read Slythe's Journal behind some barrels and the side table to start the quest.

Find and Read Slythe

Find a page of Slythe's Journal behind the two barrels and the side table and read it.

Head to Sandstone Cavern

Sandstone Cavern
Head to the Sandstone Cavern northeast of Kvatch. You may need to kill the enemies outside of the cave as they are near the cave's entrance.

Find the Body of Slythe Seringi

Body of Slythe Seringi

As you explore the cave, you will find more journal pages on the floor. These lead you to the body of Slythe Seringi at the end of the cave.

Loot Slythe's Journal, page 3 from his body to complete the quest. You can defeat the Sunken One but this is not necessary to complete the quest.
