How to Recruit Atronach Familiars
Find and Unlock Frostcrag Spire
To recruit an Atronach Familiar, you'll first need to unlock Frostcrag Spire and its Atronach Altar. Head over to the large tower perched atop the mountain to the east of Bruma and pick the front door lock to gain access to Frostcrag Spire.
Once inside, read the book placed on top of the small ice pedestal. Once you exit the book screen, the wall directly in front of you will open up, revealing the main level and the Atronach Altar.
Use 3 Fire, Frost, or Void Salts to Create a Familiar
With Frostcrag Spire unlocked, simply head on over to the Atronach Altar in the main level of the tower and interact with it. Use 3 Fire Salts, Frost Salts, or Void Salts to summon a Flame, Frost, or Storm Atronach Familiar, respectively.

Note: Only 1 Atronach Familiar can be active at any given time. In order to create a different familiar, you'll first have to "speak" to your current familiar and choose to Dismiss it.
Atronach Familiars Can be Recruited Alongside Other Companions
While you can't have more than 1 Atronach Familiar following you, it is possible to create one while also having a different companion following you already. For example, you can create a Storm Atronach Familiar while also having a Battlehorn Man-At-Arms following you around.

Of the 3 Atronach Familiars, both the Frost and Storm Atronach Familiars are smaller versions of their summoned counterparts. The Flame Atronach Familiar will be the same size as a regular Flame Atronach summon.
Unlike their summoned counterparts, Atronach Familiars are immune to player damage. If you attack them as a player, the damage is reflected back to you and it is possible to die from the reflected damage.
