Go inside the House of Healing which is northwest of the Waning Moon. Once inside, the nurse will stop you from going any further and you will have to pass a Charisma check to proceed.
Make your way to the surgery room where Malus Thorm is performing an operation on a patient. You can choose to face him in combat or ignore him completely as you'll only need to get to the office behind the surgery area.
How to Beat Malus Thorm (House of Healing)
Take the Key on the table and use it to unlock the door to your right that leads to the Graveyard.
Once in the graveyard, go up the stairs to the north and you'll end up at the entrance of the Thorm Mausoleum.
As you make your way up, Raphael will greet you at the entrance of the mausoleom. This begins the Kill Raphael's Old Enemy where he requests for you to kill Yurgir deep within the mausoleum.

How to Solve the Thorm Mausoleum Puzzle |

To solve the Thorm Mausoleum Puzzle, you'll have to light up the paintings through their buttons in this order: Moonrise Tower, Grief, and General.
Begin the puzzle by pressing the button of the Moonrise Tower painting on the southwest corner of the room.
Next, press the button under the Grief painting on the southeast corner.
The third and last painting that you need to light up is the General portrait to the north of the room.
After this, a secret passageway will open leading to a Traversal Gem elevator that takes you deep down to the Gauntlet of Shar.
The Thorm Mausoleum serves as the entry point to the Gauntlet of Shar Trials. The trials involves a series of challenges that rewards you with Umbral Gems. These gems are used to further advance through the Gauntlet of Shar and access Shadowfell.
