To get the Fortitude Perk, defeat the Vile Bilemaw and search for the Fortitude Perk hidden in the grass.
All Legendary Enemy Locations
From the Fogged Expanse meditation point, follow the path in the map above. In the last section, you'll reach a cliff where several Roller Mines will spawn all around you. Jump down off the cliff to a mudslide, where the Vile Bilemaw Legendary Enemy is waiting. Defeat it and grab the Fortitude Perk hidden in the nearby grass.

The Fortitude Perk serves as an equalizer to everyone on the battlefield. It gives the added bonus of increasing your damage output, and also provdes a challenge to avoid or defend enemy attacks.
If equipped with the Purity Perk, the perks will increase your damage dealt exponentially.
