Bloodthirst, the legendary dagger, is a weapon wielded by Orin the Chosen of Bhaal. Before you can take a crack at obtaining it, you will first need to follow the bloody trail of Orin and her Bhaalist murder cult.
Upon arriving at Rivington during the start of Act 3, head over to the Open Hand Temple. There, you can speak to Sister Yannis and ask her about the investigation into the temple murder.
Open Hand Temple Murders Guide
The easiest way to start the investigation is to go straight to the source. Dig up the corpse of Brilgor in the temple's small cemetery and cast Speak with Dead on him. Ask him about his killer as well as where he was killed to find out about the red-clad dwarven killer and the tunnels underneath the temple.
Underneath the temple will be a series of tunnels filled with massacred refugees. Kill Orin's Shapechangers and use Speak with Dead (again) on Zomm to expedite the investigation.
After a few questions, it will tell you that the Flowery Key you looted from one of the other assassins opens a room in Fraygo's Flophouse. This is the hideout of the mysterious red-clad dwarven assassin named Dolor.
Dolor's little hideout can be found at the top floor of Fragyo's Flophouse. Use the key on the Wardrobe to pass through a secret door leading into his abode. Retrieve his target list to present to Investigator Valeria.

Tip: If you are having a hard time getting onto the South Span of Wyrm's Crossing bridge due to the guards, you can actually use a short jump to access it instead. Head on over to the eastern side of the bridge, and use the small ledge shown above to jump onto the bridge platform.
With the evidence secure, head on over to Sharess' Caress (the brothel) and look for Investigator Valeria at the second floor of the tavern area. Report your findings and she should should give you a Lower City pass, allowing you to link up with Gauntlet Devella who is also investigating a string of murders in the city.
With the Open Hand Temple Murder resolved, make your way into Baldur's Gate using the Lower City pass. Since you now have a list of potential victims, all you need to do is warn them that they are on a list of murder targets (they'll be marked on the map).
Note: Informing Gauntlet Devella about the murder list is optional. She doesn't really do much with the info aside from getting paralyzed when she tries to help Figaro.
To make quick work of this step, you'll only need to save 2 potential victims. First, save Cora Highberry at the Wine Festival. Confront her about the killer to have Dolor break his disguise as a wine merchant. Defeat the 4 other Dolor copies to save Cora.
Next on the list will be Figaro Pennygood. As soon as you reach his Salon, a cutscene will play showing Dolor already having paralyzed Figaro. Intervene and kill Dolor to complete the murder investigation in Baldur's Gate.
After securing everything you need to appear before the Murder Tribunal, the next step is to actually find them. However, as soon as you set foot in the sewer area of Baldur's Gate, you will be confronted by Orin. She tells you that she has captured one of your party members and that she will kill them if you do not take out Enver Gortash first.
Now, you have to make a choice: kill Gortash before Orin, or rush Orin inside their Undercity Bhaalist temple (risking the death of your captured party member). Regardless of your choice, Bloodthirst can only be obtained once you beat Orin.
If you killed Dolor, you will have noticed that he was carrying a grotesque (and aptly named) Hand Bag. Take this to Candulhallow's Tombstones in the Lower City. Inside, take down the painting called Butterfly on Skull to reveal a button that opens the way to the Murder Tribunal.
Enter the Bhaalist outpost and present this bag to the guards of the Murder Tribunal. This secures you an audience with Sarevok. Again, you have a choice: defeat Sarevok or submit to Bhaal and kill Investigator Valeria. After you pick and complete either of the options, you will gain access to the Temple of Bhaal in the Undercity of Baldur's Gate.
Picking either side will have its own pros and cons. Becoming an Assassin will make the Murder Tribunal interaction much smoother as you only have to describe your kill before having to murder a rather squishy Investigator Valeria as your sacrifice to Bhaal.
However, killing Sarevok will reward you with the Sword of Chaos and Sarevok's Horned Helmet, on top of the Bhaal Amulet you need to enter the Bhaal Temple in the Undercity. Just be prepared to pass a Charisma Check when meeting Orin since killing Sarevok will have her attempt to kill your captured friend (even if you killed Gortash beforehand).
With the Amulet in hand, head on over to the entrance of the Temple of Bhaal in the Undercity. Present the amulet to enter and make your way towards the large open area leading to the temple. Before you can meet with Orin, you will need to pass your final test (this happens whether you become an Assassin or not).
Waiting inside this room are an army of Bhaal's assassins. All of them will be hidden and the only way to detect them is to pass a staggering DC30 Perception Check for each group of assassins. You will need to defeat Farslayer of Bhaal Ghislev to pass the test.
For players who don't want to deal with the Ambush, one nifty way of bypassing it is to simply cast invisibility for your entire party and run towards the Metal Door that leads to the Temple of Bhaal.
After dealing with Ghislev, make your way further through the cavern until you reach the Temple of Bhaal. Head inside and confront Orin at the central platform. Again, if you killed Sarevok you will need to convince Orin not to kill your captured party member.
All that's left to do is to kill Orin. Once you've dealt with her and watched the cutscene where you take her Netherstone, you will receive Bloodthirst. It will be deposited straight into the inventory of the character featured in the cutscene.
During the battle, Orin will transform into her Slayer Form, receiving a blessing from Bhaal that gives her 7 Unstoppable Charges. If you've fought some of the Bhaal assassins earlier in your run, you will know that an Unstoppable charge reduces an incoming instance of damage to 1.
For Orin, these charges refresh every turn. Naturally, this means that the only way to defeat Orin is to remove all 7 charges before the turn ends, while also dealing enough damage before the new blessing kicks in.
