The Extra Medium has a chance to drop after defeating Primordial Guardian Inceptus inside Arch of Inceptus in Dissected Plateau, Fadefields. He can be fought again by using the Moxxi's Big Encore Machine near the boss arena.

You will need to acquire the Fadefields Vault Key Fragments first before accessing the Arch of Inceptus.
