The Move Relearner, also known as the Move Maniac, can be found in the purple-roofed house a few steps east of the Pokemon Center on Two Island. This area becomes available after beating Blaine and getting the Volcano Badge.
Before the Move Relearner can teach forgotten moves to your Pokemon, you'll need to give him either 2 Tiny Mushrooms or 1 Big Mushroom for every move you want to relearn. These can easily be found as held items from Paras located in Mt. Moon.
Go to Route 4 and head into Mt. Moon via the cave entrance right next to the Pokemon Center. You can use a Pokemon that has Fly to fast travel straight to Route 4 and save time.
Once you're in Mt. Moon, head east past the girl near the entrance. Keep following the path and go down the first ladder you see along the way.
You should find yourself in a reverse L-shaped passage. Keep walking around within the area until you encounter a wild Paras. Every Paras has a 50% chance of holding a Tiny Mushroom and a 5% chance of holding a Big Mushroom.
You can use a Pokemon with Thief to steal the Paras' held item or catch the Paras along with its item entirely. Just repeat the process until you have enough mushrooms.
Return to the Move Relearner on Two Island with the required mushrooms. He should automatically offer his services, which allow your Pokemon to relearn any forgotten move from its current or previous levels!
