The choice to spare or kill Dr. Giles Rasmussen happens near the end of the A Most Extraordinary Specimen companion quest. This choice not only opens up different additional perks for Inez, but also the side quest and ending paths you will be receiving.

Apart from the ethical dilemma of justifying the abhorrent experiments, choosing to spare Giles Rasmussen is the best choice as the different grafts Giles Rasmussen provides you are miles better than the additional charge you gain from Mechanical Augment.
Furthermore, Giles Rasmussen sticks to his word and changes for the better as he fully focuses on making the grafting program an accessible and ethical means to help a lot of people.

Alternatively, Kill Giles Rasmussen for More Healing
If you were not in line with the morality of the situation, killing Giles Rasmussen rewards you by giving you more healing charges if you are in dire need of more survivability. However, the additional charge is not worth it since the fight should already be done before getting to use your secondary charge.
