How to Escape Dayton Hospital Puzzle
Go Back in Time to September 15th
The doors inside the hospital ward move you forward or back in time. Since you enter the room on 1/23, you'll need to enter the room behind the nurse's station eight times, and the door to its right four times to go back in time to your date of discharge on 9/15.

How to Start Dayton Hospital
Talk to Paul Inside the Warehouse
Start the Dayton Hospital side quest by talking to Paul in the warehouse southwest of Dayton Hospital. When you're near the warehouse, you'll hear Paul calling for help and initiate the side quest.

You can start Dayton Hospital by going to the exact location indicated above in Dayton Wetlands and interacting with Paul.

Find the Emergency Meds
After speaking with Paul, he'll ask you to look for emergency meds and his wife Kayla. Head northeast towards the hospital, and go up to the second floor where you'll find both the meds and Kayla inside a stock room.
Read the note that Kayla left behind first before taking the meds, otherwise you will be forced to the next step of the quest.
Follow the Wheelchair
After taking the meds, you'll be taken to an alternate timeline thanks to a Deviation. Follow the linear path until the dead end, then turn back and follow the wheelchair into the hospital ward.
Enter the Hospital Ward
As soon as you step inside the ward, you'll hear an announcement saying you'll be discharged on 9/15 (September 15th).
Inspect the calendar behind the nurse's station and you'll find the current date is 1/23 (January 23rd).
Travel Through Time to Discharge Date
To escape from Dayton Hospital, you'll need to go back in time to your discharge date on 9/15 or September 15th. The doors in the hospital ward will move you forward or back in time.
Assuming you haven't entered any door and are starting from 1/23, you'll need to enter the door behind the nurse's station eight times, and the door to the right of the nurse's station four times.
Escape from Dayton Hospital
When you reach 9/15, you'll hear another announcement saying you're ready to be discharged. Interact with the phone and exit through the door on the left of the nurse's station.
Capture the Space Turner and interact with the wheelchair to go back to the original timeline. Return to Paul to claim your rewards and conclude the side quest.
