Quality Nihil Blade will start to appear in chests upon reaching the upper floors of the Yabbashah Block. The Yabbashah upper floors start at 93F until 118F.
They can be found randomly in chests emitting a green light within the upper floors of Yabbashah Block and beyond.
Quality Nihil Blade can be found within Monad Door chests. Monad Doors appear randomly and contains powerful enemies guarding them.
They will also disappear when you don't access them and you move to different floors, so make sure to access them immediately while being prepared first in order to get the chest rewards inside.

Unlike Nihil Blade, the Quality Nihil Blade lets you fuse stronger weapons for the Protagonist and other party members.
Players will be able to fuse one Quality Nihil Blade weapon of their choice if they at least have one Quality Nihil Blade and other required materials.
You can fuse a Quality Nihil Blade weapon in Mayoido Antiques when you unlock it on 7/18.

You don't get Quality Nihil Blade in the floors mentioned here. It's on the floor block after the one they're saying here. I spent forever trying to find it there, but it only shows up on higher floors.
