Collect from Pillar of the Fallen near the Old Home in Teshio Ridge

From the Old Home in Teshio Ridge, climb up the mountainside nearby and keep climbing until you reach the top. The Pillar of the Fallen can be found at the top, along with the Blood in Snow sword kit.
