After obtaining Dimitrescu's Key from her Private Chambers and trying to exit the room, Dimitrescu will appear in front of you. She will smash you through the floor and you will end up in the Dungeon. Upon finding a switch, Dimitrescu will slice off Ethan's right hand.
Useless in its current state, however you must retrieve it to activate the switch leading forward. Once reattached it functions... like a hand.
